Hazard / Incident Investigation Report Template Businesses are required to investigate workplace incidents. Hazard / Incident Investigation Reports are used to formally document workplace incidents, near misses, and hazards. An effective Incident Investigation Report will record what happened, when, and who was involved; identify immediate and underlying causes; analyse contributing factors (e.g., systems, environment, behaviour); and then define corrective actions to prevent recurrence. A structured investigation process ensures that incidents are not just recorded but properly understood. The Incident Register is a central log of all reported incidents across your business. It allows you to: Track incidents over time Identify patterns and recurring issues Monitor severity and frequency Support reporting and compliance requirements Maintaining a central register provides visibility and enables data-driven decision-making. Download the Incident Report Register here. Reporting an Incident First, make sure everyone is safe, then notify Workplace Health and Safety Queensland. In Queensland, businesses have legal obligations under the Work Health and Safety Act 2011 (Qld) to notify certain incidents and take appropriate action. Serious workplace injuries include a work-related death, serious injury or illness, or a dangerous incident, at or caused by something in your workplace. First, make sure everyone is safe, then notify Workplace Health and Safety Queensland. Business owners are legally required to do so to avoid penalties.
